Nervous System Case Study
A patient has been diagnosed with a disorder of the nervous system. The patient has been
hospitalized and subsequently discharged to the care of their primary care physician. You
are the Physician’s Assistant to the MD. After the doctor sees the patient, your task will be
to answer questions about patient’s issues and write a follow-up letter to them to recap
their status, treatment plan and allay any fears they may have.
You will choose from one of the 3 case studies. Use the Case Study Report to complete
your work. Clues can be found for the diagnosis within the write-up for each patient, in
addition to their symptoms.
(1) Confirm the diagnosis and thoroughly answer the questions present.
(2) Write a follow-up letter to the patient answering the questions at the end of the case
study and allay any fears they may have. The letter should be at least one page
(300-350 words). The letter should be double-spaced.
The following 4 questions need to be answered on page 1 of the Case Study Report for any
option that is chosen. All words must be your own.
1. What is the diagnosis? How did you determine this diagnosis?
An answer of “I Googled it” or “the symptoms matched” is not sufficient – which
clues did you use? How did you put it all together? Explain your thought process.
2. What is the underlying anatomy/physiology for this disease/disorder?
How does it affect the nervous system?
Simply saying it is a “CNS/PNS disorder” is not sufficient. Be specific as you can as
to which components of the nervous system are involved and explain how they
are involved.
3. Which risk factors did this patient have? List them.
A risk factor is different from a symptom. A risk factor is something that makes
someone more susceptible to a disease (ex. age). Be sure to only list risk factors
present for the patient in question, not a list of all risk factors.
4. What tests would be performed by the hospital to determine this
diagnosis? List them.
If someone goes to the hospital, you can be pretty sure there were tests
performed to help confirm the diagnosis. List them.
Case Study #1
Claire Beauchamp is a 31-year old woman who has had episodes of severe pain on one
side of her head every day for weeks. The pain lasts for up to 30 minutes and then
subsides. She usually gets this pain during the fall or spring. Mrs. Beauchamp is a
smoker and drinks alcohol. She was having such severe pain one day that she went to
the ER for assessment. She has been diagnosed with an autonomic nerve issue – these
are her symptoms:
• Severe unilateral pain in the head
• Pain behind the orbit of the eye
• Red eye
• Runny nose
• Eyelid swelling
• Feels like a hot poker has stabbed her in the eye
Answer the four questions (listed above in the instructions) on page 1 of the Case Study
Report.
Write a 1-page letter to Mrs. Beauchamp explaining her diagnosis, her treatment options
and outlook for recovery on page 2 of the Case Study Report. You will also want to
address lifestyle factors to reduce her risk for another incident like this. Lastly, you will
want to allay any fears the patient may have moving forward.
Case Study #2
Jamie MacTavish is a 42-year old healthy male who developed a gastrointestinal viral
infection. About a week later he started to exhibit symptoms in his nervous system that
became worse and worse until he was hospitalized. He was admitted into ICU and placed
on a ventilator. He has been diagnosed with a disorder of the PNS – these are the
progression his symptoms.
• Tingling and weakness in the lower extremities
• Unsteady walking
• Tingling and weakness spread to upper extremities
• Rapid heart rate
• Low or high blood pressure
• Increasing weakness in muscles until paralysis occurred
Answer the four questions (listed above in the instructions) on page 1 of the Case Study
Report.
Write a 1-page letter to Mr. MacTavish explaining his diagnosis and his treatment options
on page 2 of the Case Study Report. You will need to be thorough with the treatment
options. Explain his outlook for recovery – can you assure with certainty that he will fully
recover? How will you be supportive while being realistic? Lastly, you will want to allay
any fears the patient may have moving forward.
Case Study #3
Bree Wakefield is a 19-year-old college student, living in a dormitory on the campus of
her university. She started to feel poorly and began to miss classes. She went to the
student health center and was immediately referred to the hospital. She was diagnosed
with a disease of the CNS – these are her symptoms:
• Fever
• Stiff neck
• Drowsiness
• Vomiting
• Severe headache
• Rash
• Light sensitivity
• Beginning stages of kidney failure
Answer the four questions (listed above in the instructions) on page 1 of the Case Study
Report.
Write a 1-page letter to Ms. Wakefield explaining her diagnosis and her treatment options
on page 2 of the Case Study Report. Explain how she can protect herself in the future
from this disease. Explain her outlook for a full recovery. Lastly, you will want to allay
any fears the patient may have moving forward.
